ADMISSION NOTICE YEAR 2015
Applications are invited from the candidates for Admission in the following Postgraduate Medical and Dental courses for the Academic Year 2015Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â (Session will start from 01.01.2015).

1)Â Â Â Must have passed FCPS Part-I during the last three years
(Only for Deputation candidates)
The following are the Guidelines for applicants to various Diploma / Degree Courses:-
1.Â Â Â Â Â Â The candidate should apply to the Principal, Postgraduate Medical Institute, 6-Abdur Rehman Chughtai Road, (Jail Road) Lahore on the downloaded Form from the Website of PGMI along with the Original Pay Order/ Bank Draft of Rs.1000/- in favour of the Principal, PGMI, Lahore.
2.Â Â Â Â Â Â Complete applications along with the Original Pay Order/ Bank Draft of Rs.1000/- can be submitted at PGMI, Lahore/ send to the Admission Cell, PGMI, Lahore through Courier Services before 20th August, 2014.
3.Â Â Â Â Â Â Government employees should submit their applications through proper channel on downloaded Forms for Deputation to the Principal, Postgraduate Medical Institute, Â Lahore. The applicants will attach a recommendation letter/ N.O.C. from the Head of the Â Â Institution / Â Â Principal/ Chief Executive.
4.Â Â Â Â Â Â All Rules & Regulations will apply as per Punjab Government Deputation Policy for Punjab Government employees.
5.Â Â Â Â Â Â Others shall apply as Private candidate on downloaded Admission Forms, for Private candidates and send / submit the application Forms directly to the Admission Â Cell, Â Â PGMI, Lahore.
6.Â Â Â Â Â Â One Admission Form will be considered for one course only. A candidate can Â Â Â Â Â Â Â Â Â Â Â apply for three courses only (along with the Original Pay Order/ Bank Draft of Â Â Â Â Â Â Â Â Â Rs.1000/- for each course). Other courses should be mentioned in the Admission Form.
7.Â Â Â Â Â Â Attach attested photocopies of the following documents with the Â Â Â Â Â Â Â Â Application:-
i)Â Â Â Â Â Â Â Â One Passport size photograph duly attested on front (paste on Admission Form)
i)Â Â Â Â Â Â Â Â ID Card and Domicile
i)Â Â Â Â Â Â Â Â Matric certificate, F.Sc certificate & B.D.S. Degree (for Dental courses)/ M.B.B.S.Â M.D. degree (Equivalence Certificate of PMDC should be attached).
i)Â Â Â Â Â Â Â Â Valid PMDC Registration Certificate.
i)Â Â Â Â Â Â Â Â Attempt Certificate from the Principal of the Medical College stating that the number of attempts and marks obtained in each professional examination of MBBS/ MD.
i)Â Â Â Â Â Â Â Â House Job certificate with exact dates from the concerned Medical Superintendent as well as Professor/ Head of the Department of concerned Hospital (Both are necessary).
i)Â Â Â Â Â Â Â Â Service certificate (relevant experience as M.O./ Registrar/ Demonstrator etc.) from the concerned Medical Superintendent as well as Principal/ Professor/ Head of the Department of concerned Hospital Medical College.
i)Â Â Â Â Â Â Â Â First appointment letter from Government of the Punjab, Health Department, Lahore as Medical Officer on regular basis.
i)Â Â Â Â Â Â Â Â Rural Service Certificate / Army service Certificate with exact dates and places where served from the concerned Director Health Services/ Competent Authority respectively.
i)Â Â Â Â Â Â Â Â M.Phil applicants will submit the result card of GAT General Test/ Admit Card at the time of interview.
i)Â Â Â Â Â Â Â Â Application with incomplete documents will not be considered for Admission.
i)Â Â Â Â Â Â Â Â Admission is liable to be cancelled during the course if any discrepancy found in submitted documents.
|Â Admission (GAT-General required for M. Phil courses)
|As per directions of the Higher Education Commission and the University of Health Sciences, Lahore, applicants seeking admission in M. Phil courses are required to pass the GAT General test with a minimum 50% cumulative score conducted by National Testing Service for registration with UHS.Applicants in M. Phil courses are directed to submit the result card of the GAT General test along with the application.
The GAT-General test is valid for a period of two years.
|Â No registration for Graduates of University of Lahore
|Graduates of the University College of Medicine & Dentistry, University of Lahore would not be considered eligible for admission in any academic program offered by UHS at the University, its constituent or affiliated institutions.
|Graduate Assessment Test (GAT General)
conducted by by National Testing Service (NTS), Islamabad. is mandatory for admissions inÂ M. Phil coursesÂ at PGMI, Lahore.
|Applicants have to submit the result card of GAT General test
at the time of submission of applications
|GAT GeneralÂ required by the
Higher Education Commission (HEC)
and University of Health Sciences (UHS).
